We were working so hard that we couldn’t keep track of the business. Billings were up, collections were down, and despite a full schedule we could barely make expenses.Clinical Management Solutions began by focusing on small, practical specifics. They discovered errors in the way we were billing, and other errors in our billing service’s practices.
They collaborated with our billing service to teach them proper practices, and monitored their actions while working accounts themselves. Old receivables were aggressively pursued and collected — or written off, if no longer collectible. Reimbursement patterns for procedures or frequent billing codes were reviewed, and compared to the cost of providing services or supplies. Required documentation for given levels of coding was reviewed and printed on our superbills; at every encounter, we could check to assure that our notes supported our coding.
Germania Echeverry held regular phone meetings with my staff — and most importantly with me. She coached us on each area that needed improvement. In-office collections improved; timely and complete charting at time of service became a realistic goal. Patient scheduling became more efficient, and we began to run — almost — on time.
We began to really look at expenses, to understand where the money was going, and to take charge of that process. We tightened up scheduling. We eliminated or referred out unreimbursed or unprofitable procedures. Where denials had been due to errors in coding, we corrected those errors. We eliminated those insurance carriers who paid less than the cost of good medicine, and by so doing were able to keep our indigent patients.
Throughout the process, CMS isolated problems, offered solutions, and insisted that we understand each step. One by one, they sewed up the holes in our pockets. We have started to get on our feet.
Currently we are bringing the billing in house. CMS has helped us evaluate software programs, and shared in purchase negotiations with us. They will help insure proper installation, and will continue to coach us as we take over one more piece of our business.
Reimbursement here in California is almost punitive. We are all still working hard. But the practice is running more smoothly, net return is up dramatically, morale is terrific, and I’m not having fantasies of simply walking away. In fact, I like my job!
